After scheduling your appointment by contacting our office, you will receive a text or email, allowing you to effortlessly complete your patient forms electronically. Alternatively, you may choose to print and fill out the patient forms provided below before your initial visit. For added convenience, you also have the option to arrive a bit earlier and complete the forms at our office

Before Your Visit, Here’s What You Should Do:

What to Anticipate on Your First Visit: 

You will be welcomed by our friendly front office staff, who will assist you in navigating the insurance process, and treatment program, allowing you to concentrate on your recovery. 

Your initial evaluation will be conducted by a highly skilled doctor of physical therapy.  

During the assessment, your therapist will inquire about your medical history, current pain or injury. Effective communication is vital for us to comprehend your concerns. 

Your movement, strength, coordination, and more will be assessed. Please don’t hesitate to communicate any discomfort or issues during the examination. 

Your therapist will outline a treatment plan designed to expedite your recovery and yield optimal results. You will have a discussion about these recommendations. 

Some treatments will be administered on your first day to kickstart your path to recovery. Subsequent visits will delve deeper into your comprehensive treatment plan. 

Appointment scheduling will be coordinated with our front desk staff, taking into account your therapist’s recommendations. We offer a range of convenient appointment times, but booking in advance is advisable, particularly during peak hours. 

Your therapist will document their findings and treatment plan, which will be shared with your referring physician or another chosen medical professional.

No-Show/Cancellation Policy: 

Please note that any treatment session not canceled at least one business day prior to the scheduled date and time will incur a $50 fee.
